Welcome to the 2025 DEAR Diabetes Educational Luncheon!

We are excited you are interested in hosting a table at our upcoming event.   In addition to our speaker lineup this year, we are encouraging vendors and organizations who support individuals living with diabetes in our community to attend and highlight their services.  

All tables cost $50 and your registration includes one attendee for the lunch.  Additional attendees may register for the luncheon through the online event registration.    

To reserve your spot, we ask that you register at least 30 days in advance.  Late registrations will be based on availability.  Your registration includes one attendee to the event. 

Terms and Conditions

You agree to indemnify and hold harmless DEAR Diabetes, Inc. and the employees and volunteers of DEAR Diabetes Inc., from any and all causes of action which may arise from the operation of this event. 

DEAR Diabetes reserves the right to accept your registration at our sole discretion.  

You grant permission for DEAR Diabetes, Inc. to use any photos, videotape, etc. taken of you or any products in any and all publicity and advertising promoting DEAR Diabetes, Inc. 

You understand that no refunds will be given due to inclement weather. 

By submitting this form and sending payment, I acknowledge that this agreement has been read and understood, and I will abide by these terms or as directed by DEAR Diabetes Inc. staff or management.

I further acknowledge that privacy and confidentiality are of utmost importance to DEAR Diabetes and our guests and I will not disclose any confidential information and I will ensure any representatives of my organization will follow these guidelines.
